APPOINTMENTS

New Patients: Your initial visit will provide you the opportunity to get acquainted with your provider and our practice. We will review your personal and past medical history, your current medications, and any medical records you bring to your visit. For the first visit, please arrive 15 minutes prior to your scheduled appointment time.  Also, please be sure you bring your insurance card with you.

Established Patients: Follow-up visits provide care for conditions that require continued monitoring to ensure progression in your health management. If you are a returning patient, please arrive 10 minutes prior to your scheduled appointment. At your return visit, our staff will confirm your information to ensure our records are accurate.
